 How to Apply  

General Application Guidelines

Pui Tak Christian School (PTCS) accepts admissions applications for the next school year on a rolling admissions basis.

 

  1. Complete the application attached below and return to PTCS via email (admissions@puitakschool.org) or in person at the school office on Mondays or Thursdays between 9:30 a.m. and 3:30 p.m. While the office is open at specified times, we suggest online applications as this will decrease your wait time.

  2. Once the application is received, the office staff will contact you to set up a meeting with the principal (in-person or video). It is advised that both the student and parent/s are present during this meeting.

  3. After the meeting, applicants will receive an acceptance letter with enrollment forms via email, or an alternative recommendation for the child's educational path.

  4.  Every applicant will go through an assessment with the Principal to determine grade-level placement. Please contact our school office immediately to make an appointment by calling 312-842-8546 or emailing enrollment@puitakschool.org to schedule your assessment. An assessment fee of $50 and must be paid on the day of the appointment.

  5.  There is limited space in our classrooms. You are encouraged to apply early to secure a spot.

Please Note: Upon acceptance, to guarantee a place, a one-time $50 registration fee and one month tuition deposit will be required. These fees are non-refundable, though the tuition deposit will be applied to the last month of attendance.

 Preschool  

 

For preschool students, we accept applications at anytime. We encourage parents who want to enroll their child to apply as soon as they can (child must turn 3-years-old by September 1st of the school year requested). If your application was submitted by November 1st, your child will take part in the group assessment day in early December. In November, parents will receive a phone call scheduling an assessment appointment on the group assessment day. By the end of January, parents will receive an acceptance packet from the school office. Once the paperwork and fees are turned in at the office, your child's enrollment is finalized. If your application is submitted after November 1st, your child will have an assessment with Principal Ho individually. A $50 assessment fee applies for all applicants, check or cash only.

 Elementary K-8  

 

Kindergarten, elementary and middle school students’ application are received on a rolling basis. We accept applications throughout the year. Once parents send in an application, the school office will schedule an assessment time for the child and for the parents to meet with Principal. The assessment fee of $50 applies.

Pre K Full day parents should:

1. Register by completing the appropriate enrollment packet and returning it to the school office along with the registration fee ($50) and the one-month-tuition deposit, both non-refundable.
2. You also need to bring along your Birth Certificate at the time of registration. A student's enrollment is not confirmed until the above fees have been paid and the forms have been received by Pui Tak Christian School.

Pre K Half day parents should:

1. Register by completing the appropriate enrollment packet and returning it to the school office along with the registration fee ($50) and on-month-tuition deposit, both non-refundable.
2. You also need to bring along your Birth Certificate at the time of registration. A student's enrollment is not confirmed until the above fees have been paid and the forms have been received by Pui Tak Christian School.

K-8 parents should:

1. Register by completing the appropriate enrollment packet and returning it to the school office along with the registration fee ($50) and the one-month-tuition deposit, both non-refundable, within five working days.
2. You also need to bring along your Birth Certificate at the time of registration. A student's enrollment is not confirmed until the above fees have been paid and the forms have been received by Pui Tak Christian School.
